Forms for New and Existing Vendors
Vendor companies are required to complete the following upon award:
W9 - Request for Taxpayer Identification Number and Certification Form W-9 (Rev. January 2011)
Independent Contractor Forms for the University (complete list) Please review which form should be completed with your Project Manager or primary JHU contact.
- Independent Contractor Certification - For Services of $5,000 or more
- Short Form Independent Contractor Agreement - for Services less than $5,000
- Independent Contractor Agreement - For Short Duration Services of more than $5,000 and less than $50,000
- Contractor Outage Request Form - Please complete the online outage request form including the project manager’s name, phone number, and email, as well as all other relevant outage information. The form will then route directly to the Project Manager for review and final completion inclusive of the WBS and accounting information.
Becoming a Vendor for JHU
The Office of Facilities Management does not have a vendor pre-qualification system and welcomes all contractors, vendors and professional service firms to submit a letter of interest and company portfolio to: info.dc@jhu.edu
We encourage you to provide updated company profile information every two years. This does not guarantee that you will be solicited or awarded a particular project, but ensures that we are kept abreast of new developments and accomplishments for your company.
Request for Proposals/Invitation to Bid:
JHU policy requires that all purchase orders with a total aggregate of $5,000 or more be competitively bid. Proposers and bidders are by JHU invitation.
We invite proposals/bids on a project-by-project basis depending on size, complexity, and timing. We may provide a list of preferred vendors to a general contractor or design firm, but will gladly consider others, given adequate notice to check references.
Evaluation Criteria:
Our primary goal is to maintain high quality work while obtaining the best value (not necessarily the lowest price) for our clients and for the University. Therefore, it may not be practical to award a contract for competitive reasons alone. Your proposal/bid for a project shall be evaluated against both qualitative and quantitative criteria.
We value design consultants, constructors, and special service vendors, and specificially the individuals within those companies, who have tangible experience in the type of project being developed. We strive to maintain positive working relationships with firms that have provided outstanding service to the University in the past. We also look for opportunities to collaborate with new companies, including those with DBE certificatin. Reference checks, including constructors, consultants and other clients are valued in the selection process.
